[PATCH 4/4] Make it clearer that conflicts can be for other reasons than personal interests

The policy says that the appearance of a conflict and a duality of
interests should be disclosed, but the actual form and the intro only
talk about private or personal interest, which is a much more narrow
definition.  Make it clear that appearance of a conflict and duality
of interests should be disclosed as well.
